All forms must be completed and turned in 1 week prior to the start of practice.  Fall tryouts begin 

Aug 13th for the High School,

 and Aug 28th for the Middle Schools.

1. Go to pvvikings.com

2. Registration (right side of page) Click her for visual guide

3. Create an account **NOTE - Password must be STRONG

4. Create Parent profile

5. Click Student Profile

6. Click Add Student/ create Profile

7. Click Forms  

8. Complete all required form fields

9. Download and print the attached PIAA HH & PE Forms **

1. Fill out the HH(Health History)

2. Take HH & PE(Physical Evaluation) forms to your doctor’s appointment (after June 1, 2018).  Physician must fill out, date and sign the PE form.

10. Download and print the Emergency Info Form. **

11. Submit online registration and return the PIAA HH, PE, and Emergency Info form to the Athletic Department 1 week prior to the starting date for practice/tryouts. Original required.
