If you have obtained a report in the past, it must be dated within one yearof your start date and must show EMPLOYMENT (not volunteer) as the purpose of the report.
1) The Pennsylvania Criminal History Clearance Report is obtained electronically from the Pennsylvania State Police website: https://epatch.state.pa.us The $22.00 fee for this report is paid by credit card or debit card.
2) On the website, select “Submit a New Record Check”.
Note: You will be required to enter your information twice, once as the person requesting the report, and once as the person you are requesting the information for. After the second submission, click “Submit request” to check out.
3) Access your official certification form as shown here:
DO NOT send the unofficial page/receipt (above) to the HR department for your personnel record; it is not valid! You MUST print out the official certification form (see sample below):
4) Submit the official Certification Form to Karen Zucker in the HR Department (email to firstname.lastname@example.org or mail/drop off at the district office).
THE COST OF THE CLEARANCES WILL NOT BE REIMBURSED BY THE DISTRICT AND ARE REQUIRED OF ALL STAFF. According to state law, failure to obtain new clearances prior to any expiration dates may result in disciplinary action, up to and including dismissal from the school district.
