Nursing Health Information

  • Physical Examinations- The PA Department of Health Services requires students upon original entry into school, sixth grade, and eleventh grades to obtain a Physical Examination.
  • Dental Examination – The PA Department of Health Services requires students upon original entry into school, third grade, and seventh grades to obtain a Dental Examination.

Dear Parents,
 
Medication permission forms for the current school year can be found on the district website.  Click here to access the PVSD Medical Forms Webpage. New forms must be completed each school year and accompany the prescribed medication according to PA law and district guidelines.
